Provide your personal details as they appear on your government-issued ID. This is your individual financial statement — all information should reflect your personal finances, not business entities. No SSN or date of birth is required.
Step 2: Personal Assets
List assets held in your personal name only. Investment real estate held in LLCs or trusts is captured in the separate Schedule of Real Estate Owned (SREO) — please submit both if you own investment property.
Assets that can be quickly converted to cash. Add multiple entries per category if you have accounts at different institutions.
Non-liquid assets held personally. Include your primary residence value here.
Step 3: Personal Liabilities
List debts and obligations in your personal name. Business debts should go on your Debt Schedule. Real estate mortgages on investment properties go in your SREO and will be reflected here automatically.
Step 4: Annual Income
Report your annual personal income below. Include W-2 wages, 1099 compensation, dividends, interest, and any other personal income you receive. Alimony, child support, or separate maintenance income need not be revealed if you do not wish to have it considered as a basis for repaying this obligation.
Step 5: Other Business Ventures
List operating businesses and private business interests you hold. Do NOT include LLCs that exist solely to hold investment real estate — those belong in your Schedule of Real Estate Owned (SREO).
